Fill out your order form completely, include your charge card number and expiration date at the bottom of the form and sign it. Our fax number is 563-873-3647.

Mail Order:

Fill out your order form completely, include your personal check, money order or charge card number in the provided space. Please include shipping.

Add items from this site to your shopping cart, then click the *Process Order* button to be taken through the checkout process.

Damaged Shipments:

UPS shipping losses will be replaced only if you follow these steps:

1.  If a package is received in damaged condition, refuse it and notify us. UPS will return the damaged package to us and we will reship.

2.  If loss or damage is found after package is opened, hold all items, packing and shipping carton. NOTIFY US. We will explain how you can put in a claim with a UPS representative. Do not destroy packing or carton, as UPS will not honor any claim if carton cannot be inspected.

International Shipping:

Shipping charges for destinations out side the U.S. are estimated and charged at the time the order is placed. However if the actual shipping cost is higher the customer may be recharged for the balance of shipping due. We normally ship international packages by US Parcel Post if you would prefer your order to be shipped to you via UPS or Fed Ex Just type in your request in the 'other Information' box on the bottom of this page. Additional shipping charges will apply. No Next Day Air, Second Day Air or 3-Day Delivery is available for International orders.
